Just a reminder for all parents to complete all the necessary forms by Friday, September 16, 2016.
Online Approval Forms are found in each parent’s Edsby Account by clicking on the message envelope on the top left-hand corner of the Edsby screen. The online forms are as follows:
- Acceptable Use of Technology Form
- Media Waiver
- Crestwood Parent Association Directory Opt In
- Lunch Time Departure – Grade 10, 11 & 12 Students
Hard copies of these forms can also be found on the documents page on the school website and can be returned to the homeroom teacher as a replacement for online approval if desired.

Paper Forms were mailed out the last week of August and need to be returned to the Homeroom teachers. The paper forms are as follows:
- Student /Family Information Form
- Student Code of Conduct Form
